Content Partnership Agreement

UpperCervicalCare.com • 3-Month Video Program

This Agreement is entered into between Upper Cervical Care ("Company") and the Participant identified in this form ("Participant"). All terms are set forth below.

1. Purpose

Participant agrees to create video content to support the mission of increasing awareness of upper cervical care and connecting patients with providers through UpperCervicalCare.com.

2. Term

This Agreement is for a 3-month term, beginning on the Agreement Start Date entered in this form, with an option to renew upon mutual agreement.

3. Deliverables

Participant agrees to:

Create 8 short-form videos per month (under 60 seconds, vertical format) — 24 videos total over 3 months
Follow provided topics, hooks, and guidelines
Submit all raw video files to the designated Drive folder by the 15th of each month for the upcoming month's content

Monthly Breakdown:

6 videos — Patient-focused (health conditions)
2 videos — B2B (promoting UpperCervicalCare.com)

4. Company Responsibilities

Company will:

Provide topics, hooks, and direction
Handle editing, captions, branding, and thumbnails
Distribute content across social platforms and UpperCervicalCare.com
Tag Participant where appropriate

5. Compensation

$75

Per Video

3-Month Total

6. Participation Requirements

Participant must maintain:

Level 2 Enhanced Membership on UpperCervicalCare.com
Active enrollment in Profile Drive ($225/month)

7. Content Rights

Participant grants Company full rights to edit, publish, distribute, and promote all submitted content across social media, the website, ads, and marketing materials. Participant may repost and share content on their own channels.

8. Termination

Either party may terminate with written notice. Company retains rights to all previously submitted content. Participant will be paid for all completed work.

9. Independent Contractor

Participant is an independent contractor, not an employee of Upper Cervical Care.

10. Electronic Agreement & Submission

This Agreement may be executed electronically and is legally binding upon digital signature. Video submissions will be completed via a designated online Drive folder provided by Company.

11. Entire Agreement

This represents the full agreement between the parties and may only be modified in writing signed by both parties.
